Beloved in the Lord,

The Diocesan Office has received a significant number of Baptismal and Marriage transcripts containing numerous errors. Moreover, we have received a number of requests for the format of these transcripts in Greek.

In response, I am enclosing for your use a description of the transcript preparation and verification process which contains samples of the forms for your reference. Whenever you prepare transcripts, the wording should be exactly as indicated thereon.

Also, please note that all transcripts are original letters; not fill-in-the-blank forms; they should be typed as original letters.

Each transcript must also contain the parish seal, next to your signature.

Remember that the diocesan hierarch is required to verify the priest's signature. You therefore do not need to put a date in the final paragraph, since I will affix this when I sign the transcript or certificate; as shown, you may leave a blank line for the date.

Note that on the forms I have highlighted in bold lettering the data that would be variable from letter to letter; do not use bold lettering when you submit transcripts or certificates to my office.
